Amiodarone Infusion: Indications for Use, Administration Protocol, and Important Contraindications

Introduction

Amiodarone was originally synthesized and tested as an anti-anginal drug in the 1960s, but its antiarrhythmic properties were later discovered. In addition to its superior efficacy as an antiarrhythmic drug, it has minimal negative inotropic effects, making it advantageous for use in patients with heart failure.

Infusion Protocol
Total infusion duration is 24 hours.
– 150mg bolus over 10 mins
– 1mg/min for the first 6 hours
– 0.5mg/min for the next 18 hours

Indications of Amiodarone
In atrial arrhythmias (A Fib, A Flutter)
In ventricular arrhythmia (malignant ventricular arrhythmias and commonly.
Used for secondary prevention of recurrent ventricular arrhythmias.

Contraindications of Amiodarone
Patients with second or third-degree heart block who do not have a pacemaker.
In preexcitation (Wolff-Parkinson-White Syndrome, concurrent atrial
fibrillations)
Baseline QTc Prolongation
Hypersensitivity reactions to amiadarone
Hyperkalemia
